Consultation checklist

Print this page and take it to the consultation. Ask for the answers in the written quote. These questions do not tell you whether an implant is suitable. That is a clinical decision.

  1. 1. Does this price include the implant and crown? Ask for the implant, the abutment and the crown to be listed separately in the written quote.
  2. 2. Is the consultation included? Ask what the consultation costs and whether any deposit is refundable.
  3. 3. What will scans and X-rays cost? Ask which scans are in the quote and whether a later scan could cost more.
  4. 4. Could I need a bone graft or a sinus lift? Ask whether your plan needs one, and for an itemised cost if it does.
  5. 5. Will I have a temporary tooth or bridge? Ask what is planned between appointments and whether it is included.
  6. 6. Who will place the implant? Ask for the name of the dentist who will do the surgery, and where they are registered.
  7. 7. What material is the final crown or bridge? Ask for the material, and the number of teeth, to be written into the plan.
  8. 8. What does any warranty cover, and what would end it? Ask for the written terms, the duration, the exclusions and any visits you must attend.
  9. 9. Are follow-up visits included? Ask how many reviews are included, for how long, and what you would pay after that.
  10. 10. When is each payment due? Ask for the payment schedule, any finance charge, and what happens if the plan changes.
